Thankfully, the days of feeling overwhelmed by the passport application process are long gone. Thanks to the Department of Foreign Affairs (DFA), getting your passport has never been easier.

How to Book a DFA Appointment

Booking your passport appointment online requires a stable internet connection.

To apply for a Philippine passport, you need to book an appointment online for your in-person visit at the DFA first. Review the passport requirements (see below) before finalizing a date to give yourself ample time to prepare the documents.

 

Schedule an appointment by following the steps below. Note that the system will reserve your time slot for 30 minutes only. So, get a good internet connection to avoid lag or interruptions while completing the passport online form.

 

  1. Go to passport.gov.ph and click "Schedule an Appointment."
  2. Tick the terms and conditions checkbox and select "Start Individual Appointment."
  3. Pick your preferred branch from the list of consular offices and click "Next" when done.
  4. Check the earliest date available in green font or highlight.

Switch your DFA branch to find an available time slot if aiming for a specific date.
  1. Select a time slot after choosing a date.
  2. Fill out the passport application form with your personal information. Remember to provide an active email address.
  3. Pay the passport fee of ₱950 via GCash. You can get your passport in 10 to 12 days.

 

For a faster processing time, pay ₱1,200 to receive your passport in six working days in Metro Manila or seven days outside of it. An additional shipping fee applies for home delivery.

 

After payment, you should receive an email confirming your in-person appearance. The email will contain your completed passport application form showing your date and time slot. Print the form and bring it to your passport appointment along with other requirements.

Passport Application Requirements

 

Give yourself two to three weeks to prepare these requirements before your scheduled appointment.

 

Adults (18 years old and above)

 

  • Printout of completed passport application form
  • Original copy and photocopy of Philippine Statistics Authority (PSA)-authenticated birth certificate on security paper
  • Valid ID and a photocopy of said ID, such as the National ID, LTO-issued Driver's License, and Social Security System (SSS) Card
  • Original and photocopy of PSA-issued Certificate of Marriage on Security Paper or Report of Marriage for married females using their spouse's last name

 

If your PSA birth certificate is blurred, get a copy from the Local Civil Registrar.

 

Bring additional documents, such as another government-issued ID, if you don’t have an existing birth certificate or have discrepancies in your birth certificate. You can also submit the following documents.

 

  • NBI Clearance (valid or expired)
  • School records such as Form 137-A, transcript, or diploma obtained from Elementary, High School and/or College
  • Service record if you're a government employee
  • Member Data Record (MDR) from PhilHealth

Minors (under 18 years old)

 

Minor applicants are required to schedule an appointment online. Upon confirmation, the applicant must personally report to the DFA, accompanied by either a parent or an authorized adult companion.

 

The following documents must be presented during the on-site passport appointment:

 

  • Printout of completed passport application form
  • PSA birth certificate
  • School ID (if applicable)
  • Passport or a valid government-issued ID of either or both parents, with a photocopy of said ID
  • Special Power of Attorney executed by the child's mother if the parents are unmarried and the mother is not accompanying the child

 

Additional requirements may be needed if the applicant is a newborn or not accompanied by the parent(s) during the on-site appointment.

 

Passport Renewal Requirements

 

Applicants renewing their Philippine passports must likewise set an appointment online to personally report to the DFA. Bring the following documents for passport renewal:

 

Adults (18 years old and above)

 

  • Printout of completed passport application form
  • Current passport with a photocopy of the passport's data page
  • Valid ID together with a photocopy of said ID

 

You only need to submit a PSA-issued birth certificate if you had a name change or had your name corrected through a judicial order. If that's the case, DFA requires an original, PSA-authenticated copy of supporting papers, such as: 

 

  • Certificate of Marriage
  • Annotated Certificate of Live Birth
  • Annotated Certificate of Marriage to show annulment/divorce/court-ordered instruction
  • Certificate of Death of Spouse

 

If you're married and keeping your maiden name, you don’t have to submit a Certificate of Marriage.

 

Minors (under 18 years old)

 

For the Philippine passport renewal of a minor, the child and an adult—either a parent or someone authorized—must be present. 

 

  • Current passport with a photocopy of the passport's data page
  • Certificate of Live Birth issued by the PSA to establish proof of relationship
  • One valid ID for both the minor applicant and the accompanying adult

I need a passport asap. Can I walk in at any DFA office? 

 

DFA accepts walk-in passport applications via the Courtesy or Special Lane. Filipinos eligible to use the lane include:

 

  • Minors
  • Persons with disability (PWD)
  • Pregnant women
  • Solo parents
  • Senior citizens
  • Overseas Filipinos
  • Muslim Filipinos participating in Hajj Pilgrimage
  • Individuals with exceptional or emergency cases

 

To access the Special Lane privileges, bring proof of eligibility, such as a Senior's ID or an ultrasound report if you’re pregnant. You’ll also need to fill out this passport application form.

 

Individuals accompanying seniors, PWDs, or minors can use the Special Lane for their passport applications. However, they must provide documentation establishing their relationship with the person they accompany.

 

Where do I get the Special Power of Attorney form?

 

In addition to the completed passport application form, the email confirming your DFA appointment will have two other attachments: the Special Power of Attorney form and the Letter of Authorization for passport release. If you didn’t receive them, download the forms at consular.dfa.gov.ph/list-of-consular-forms/.

 

How do I get a passport replacement if I lost or damaged it?

 

An appointment is required for a lost passport. You must submit the necessary documents for the application process. If your passport is either valid or expired, you’ll need to provide a police report (in English) and a notarized affidavit of loss.

 

You'll also have to pay ₱350 as a penalty fee for a lost or damaged passport. The DFA will implement a 15-day clearing period for lost valid passports.

 

Can additional pages be added to the passport if it’s full?

 

Unfortunately, you can only get 44 pages in your passport. You can opt to renew your passport if you run out. 

 

I just got married. Can I still use my maiden name when I renew my passport?

 

Yes, you can! Just submit a Marriage Certificate to update your marital status from "single" to "married." In fact, you can revert to your maiden name at any point with supporting documents. 

 

When should I renew my passport?

 

It's time to renew your passport if it has less than one year before expiration. You can’t travel outside the country if your passport has less than six months of validity.
