How to Make a Claim

Submit your claim in three easy steps. You’ll receive a Claims Decision letter within 15 working days.

Report the incident immediately or within seven days through any of the channels below


Step 2

Download and complete the Gadget Care Claim form.

Step 3

Prepare the required documents below:

FOR ACCIDENTAL DAMAGE CLAIM

  1. Two latest Globe Statements of Account showing the Gadget Care Plan Monthly Recurring Fee
  2. Any proof of payment of Globe account's outstanding balance covering the claim period
  3. A photo of the damaged device/phone


FOR THEFT BENEFIT CLAIM

  1. Two latest Globe Statements of Account showing the Gadget Care Plan Monthly Recurring Fee
  2. Any proof of payment of Globe account's outstanding balance covering the claim period
  3. Original copy of Police Report secured within seven days of discovery.
  4. Notarized Affidavit of Ownership and Loss Undertaking (National Telecommunications Commission Form for Phone Blocking)
  5. Photocopy of two Valid IDs
